The website mygreen.clinic is owned and operated by My Green Clinic Pty Ltd ABN 746 876 123 71. My Green Clinic clinical operations are provided by My Telehealth Medical Services Pty Ltd ABN 86 667 533 388 who provide clinical and administrative support to patients of My Green Clinic.

Personal data and information that is provided to My Telehealth Medical Services Pty Ltd is shared with the owner My Digital Health Group and its associated entities (My Weight Loss Clinic, My Telehealth Clinic, Script2u) for the purpose of providing you with medical and administrative support services and other services to support your healthcare.

• Staff means people employed by us or contracted to us to provide the Services.
• Health Care Practitioners means Nurse Practitioners, registered nurses and enrolled nurses and other clinical staff.
